Interested in exploring a new Plan Design?

  1. Employee Census Form: If you are interested in exploring a new Plan Design, please fill out this online form completely or print it, fill it out and fax it to our office.

  2. Elizabeth Clark then submits your census to our third party consultants for options in potential plan designs.
  3. Elizabeth will contact you within two business days with your options and discuss any possible implications for each plan design.
  4. If necessary, the information will be resubmitted with any changes or amendments so that you can be sure you are receiving a customized plan for you and the needs of your practice.

Adding a new participant?

If you have a new participant, please download and submit the following documents by mail for each participant. We must have original signatures.

Each employee must submit completed paperwork with each contribution.

Include with these forms either a contribution or a record-keeping fee of $50 (fifty dollars) for each participant.

All plans are invested on the last day of every month.
